Evaluation of the Usefulness of CINE CT in Pediatric Tracheomalacia Diagnosis

Abstract
Purpose: In case of pediatric patients, unstable respiration makes it difficult to diagnose diseases such as Tracheomalacia in conventional chest CT scan. The purpose of this study is to evaluate the usefulness of Tracheomalacia inspection, using CINE technique which can measure dynamic information.

Material and Methods: From October, 2017 to January, 2018, 10 pediatric patients who were suspected to have airway disease were included in this study. We used REVOLUTION CT system(GE Medical System, USA) with a detector size of 16 cm. The imaging parameter was as follows: KV 70, mA 120, DFOV 32 cm, a rotation time of 0.28 sec and a total rotation time of 2.8 sec. The examination was repeated 10 times in cine mode, including the airway of the pediatric patients. CTDIvol and DLP were used to evaluate the doses of Routine pediatric patients and CINE pediatric patients, and noise levels were compared by designating region of interest(ROI) in both lung and trachea. We also reconstructed acquired images into Minip, Raysum, and virtual images of volume rendering.

Result: The average CTDIvol of the routine pediatric patients was 0.6mGy and DLP of them was 15mGy. The average CTDIvol of the CINE pediatric patients was 4.34mGy and DLP of them was 52.02mGy. Noise values of both lung and trachea were measured in Routine pediatric patients and CINE pediatric patients. The mean values of Rt lung were 32.88 and 32.89, respectively. The mean values of trachea were 48.72 and 51.39 and those of Lt lungs were 38.50 and 28.27, respectively. Statistical analysis was not statistically significant by p>0.05.

Conclusion: The application of the CINE CT to the pediatric airway examination makes it possible to do dynamic examinationof the airway regardless of the respiration. Also, it is expected to be clinically useful for diagnosis of stenosis or obstruction.
